REGISTER

1. Acknowledgment of Risk
I, the undersigned participant (or parent/guardian of a minor participant), understand that participation in Youth Skills Champ events involves physical activities that carry inherent risks, including but not limited to falls, collisions, and other injuries. I acknowledge that I am voluntarily participating at my own risk.

2. Release of Liability
I hereby release, waive, and discharge Youth Skills Champ, its organizers, sponsors, volunteers, and affiliates from any and all liability, claims, or causes of action arising from participation in the event, including injuries, property damage, or other losses.

3. Medical Authorization
In the event of a medical emergency, I authorize Youth Skills Champ staff to seek necessary medical treatment for myself/my child. I understand that I am responsible for any medical expenses incurred.

4. Media Release
I grant permission for Youth Skills Champ to use photographs, videos, or other media of me/my child for promotional and marketing purposes.

5. Agreement to Terms
I have read and understood this waiver and agree to its terms. By signing below, I voluntarily accept these conditions.
